I think its italian movie, i am not sure..
Rich guy middle age has "crush" with girl who have maybe 18-20 years
She fall in love with him and start to to persecute him.
He has a long relationship with girl same ages like him, i remember that he gave her present " russian babushka"
After that he was with that younger girl.. and they broke up
On the end of film he waits her on island, and she came there..
